If you require any vaccinations relating to foreign travel, please remember we do not have last minute appointments. You must complete a travel health questionnaire at least 8 weeks before departure and hand this in at Reception. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. You will be contacted by the surgery within 7 to 10 days after completion with an appointment if needed. Please ensure you put your contact number on your completed questionnaire.

Some travel vaccines may only be available at a private clinic. You will be informed of this after completion of your questionnaire.
